Accident and Incident Investigation Training.

During this DNV training course, the entire process of reporting, investigating and handling a variety of adverse events (such as accidents and incidents) is covered systematically.

Content

During this training, the entire process of reporting, investigating and handling all kinds of undesirable events (such as accidents and incidents) is covered systematically. Each step in this process and the related critical success factors are discussed in detail. Much of the training consists of practical exercises and case studies. The nine most commonly used investigation methods are covered. Two techniques are also extensively practiced on the basis of a number of cases. Participants are provided with B-SCAT software for the analysis and reporting of incidents. This software can also be used free of charge for a month and a half after the training.

  • The following research methodologies: Why-analysis; Change-analysis; Ishikawa (fishbone) diagram; Systematic Incident Reconstruction & Evaluation (SIRE); Energy and Barrier Flow/Blind Badger Model, Tripod; Prevention & Recovery Information System for Monitoring & Analysis (PRISMA); Fact Tree Analysis Technique; Systematic Cause Analysis Technique (SOAT) and Barrier-SCAT (B-SCAT)
  • Interview techniques and obtaining factual information and evidence
  • Fire: origin and development
  • Reporting and communication
  • Follow-up and analysis of incidents
